Complete Your Health Questionnaire

Pre-Draw Preparation

  • Complete your required health questionnaire, which can be found in the 'Health Questionnaire' section of your portal, when logged into your Lifeforce account. This information helps your clinician prepare for your upcoming telehealth visit and ensures that your recommendations are tailored to your unique needs. 

Select Your Blood Draw Location

In-Home Phlebotomist Booking

  • For those opting for comfort and privacy, schedule an appointment with a skilled phlebotomist to have your blood drawn at home or in the office. Make sure your address details are accurate. A phlebotomist will reach out to confirm your appointment, so responding to their message to confirm your personal information is essential.
Note: On-site blood draws are strictly available at your home or office only. Alternative locations such as hotel rooms or other public spaces will not be accommodated.

Lab Walk-In

  • Prefer the walk-in option? Simply download the lab requisition form from your portal before visiting your local Lab, as indicated in the 'Upcoming Blood Draw' section of your portal, while logged into your Lifeforce account. Fasting and Restrictions

  • A crucial part of preparation is fasting for 10 hours before your appointment and avoiding certain substances like alcohol, aspirin, and vitamin B3. This ensures the accuracy and reliability of your test results.
