Sign in to Your Portal
- Begin by logging into your Lifeforce portal, your central hub for appointment details and subsequent actions.
Download Your Requisition
- When logged into your Lifeforce portal, you'll find a prompt titled 'Upcoming Blood Draw – Download Lab Requisition' on the main page. Click to download your requisition. As an alternative, your requisition is also accessible via the 'Appointments' menu.

- Printing Is Essential: It's necessary to download, print, and physically present the lab requisition to at each visit.
- Unique Order Numbers: Each blood draw creates a new lab requisition with a unique order number. The order number helps Lifeforce receive and correctly link your results. Please ensure that you schedule your blood draw and use your most recent lab requisition before each blood draw to ensure a seamless experience.
